2.  After PHA online is complete:
   Put in request on SEC JAX Medical Request tracker at: 
   Sector Jacksonville | Medical | Microsoft Teams
   
   Once done, we will contact you once you are able to proceed to contact QTC for an appointment.
   *If you are unsure of what other services you may need to be completed along with your PHA please state that in your request and we can input that information to you. 
    
3. Contact QTC:
   Contact QTC at 1-833-782-7477 to register and submit orders. 
   OR
   Register online at https://scp.qtcm.com 

Once this is all complete and you have registered and submitted your order request with QTC, a provider will contact you to schedule an appointment.

You may call Mayport at 904-270-4401 to conduct a PHA while on active duty.

ASQ is to be completed in DA. Below is the guidance for your reference. In the event there is no DA login ability, a CG-3799R can be submitted.

Must be submitted annually.
